Stability 

  • Consider post resection 
  • Laminectomy 
    • Immature: Causes instability – arthrodesis 
    • Adult: If <50% of facets resected you are okay 
  • Vertebral Body 
    • Resection creates instability 
    • Currettage – bone grafting is okay 
    • Indications for Fusion in context of pathologic fracture 
      • 50% violation 
      • Segmental Kyphotic angle >20 degrees 
      • Involvement of Anterior and Posterior Columns 
  • Posterior Structures 
    • Bridwell says 25% contribution for each posterior stabilizing structure 
      • Midline osteoligamentous complex 
      • Facet joints x2 
      • Posterior vertebral wall/ disc/ annulus 
    • 50% violation requires fusion
